    You will hear back from us via email and be able to submit the P.O. as an attachment after that.   The email used to submit  Purchase Orders is isarllc@yahoo.com.   We can send you an Invoice, or you can submit your Purchase Order.

     The Purchase order should include the number of Call Balls you want, the shipping address, the email and physical address of the financially responsible party (usually your Accounts Payable Department), and the name and department for the point of contact to receive the Call Balls (usually the ED Director or Hospital/Facility Safety Director). 

     Most hospital or health care organizations will require a W9 from  us to establish our vendor status with them.   The W9 will be provided by upon request from your Accounting or Purchasing Department.
